Admissions process
We welcome families and support coordinators seeking a nurturing and engaging day habilitation center for individuals with mental challenges and disabilities. Our program runs Monday through Friday from 9AM to 3PM, offering more than just a day program—we build connections and foster growth.

| Schedule a tour | After receiving your information request, a member of our team will contact you to schedule a personalized tour of our facility. This is a great opportunity to see our program in action and ask any questions you may have. |
| Enrollment & support | Once you've completed your tour and are ready to move forward, we will guide you through the enrollment paperwork and answer any remaining questions about our services and support. |
Funding your loved one's care
We understand that funding is an important consideration for families. Keys To The Heart LLC accepts Medicaid/DDD for our day habilitation services. We are here to assist you with any questions regarding payment and eligibility.
